Prognostic Factors Influencing the Functional Outcome of Total Hip Arthroplasty for Late Infection Sequelae

Abstract
Purpose: To investigate the factors influencing the functional prognosis of patients having undergone total hip arthroplasty (THA) for late sequelae of infective arthritis of the hip.

Materials and Methods: The clinical and radiographic data of 91 patients (91 hips), with pyogenic or tuberculous infection of the hip joint, who later underwent THA, were retrospectively analyzed. The average follow-up period was 5.8 years. For all patients, the preoperative range of motion, leg length discrepancy and radiographic data of acetabular dysplasia and femoral canal narrowing were compared and analyzed using the assessed Harris hip scores.

Results: The fused hips had less improvement in their Harris hip score than the non-fused hips. An initial leg length discrepancy of more than 1 inch, radiographically severe acetabular dysplasia (Crowe Group IV) and femoral hypoplasia, as reflected by a 20% smaller femoral isthmus cortical width, were related with less improved results.

Conclusion: The clinical outcome of total hip arthroplasty for the late infection sequelae was associated with an initial leg length difference, fusion of the hip and severe dysplastic acetabulum or femoral hypoplasia.
